- All students entering 6th grade for the 2010-2011 school year are required to turn in a physical. These physicals should be turned in no later than December of the school year.
- All students entering the 7th grade are required to turn in a dental exam. These too, should be turned in by December of the school year. Seventh grade students must also have proof in the health office of obtaining the immunizations Tdap and Menactra. Failing to do so will result in exclusion from school. The immunizations should be of file in the health office by the start of the school year.
- Letters will be sent home via mail for all students. The required paperwork will be attached.
- Please be advised that the school board policy states, "Students who fail to complete and/or submit acceptable evidence of required medical examinations or dental screenings within the appropriate time period will not be admitted to school the following school year unless or until acceptable proof of compliance is received. "
- If you are unable to have the exams performed by your own family physician, the school will provide the exams free of charge on certain dates during the 2nd half of the school year.
